God had created every part of our body for a purpose. The surgery for anal fissure include slitting / cutting the lower 2cm of the internal anal muscle in order to relieve the tightness and spasm of the anal opening which is found in anal fissures. If there is a way to correct this spasm without even that slightest cut, then it is definitely beneficial and would be considered by all patients if they are given a choice of surgery or no-surgery remedy.
This is where Botulinum Toxin (popularly known as BOTOX® is useful.
Botulinum toxin type-A is derived from the bacteria Clostridium botulinum. This chemical, which is available as a purified freeze dried powder, causes temporary relaxation of the muscle into which it is injected under anaesthesia. The effect lasts for upto 6 months which is enough time for the anal fissure to heal in 80-88% patients. A repeat dose of BOTOX is advised to further increase the chances of cure to upto 96% in patients who did not get relieved in the first dose sitting. It is proven world wide through many medical evidence based study trials that is effective and is better than medicines for anal fissure.
The British & American colorectal surgeons’ societies have mentioned in their published guidelines to the surgeons that BOTOX is a first line treatment for anal fissures before taking a call for surgery.
Administration of botulinum toxin has a low rate of adverse reactions and complications.
Botulinum toxin is officially used in clinic for the treatment of muscular hyperactivity (strabismus, blepharospam, cervical dystonia, diffuse esophageal spasm, anismus, oropharyngeal dysphagia, anterior rectocele, and secondary achalasia). Botulinum toxins are also used in non recognized clinical applications: neurogenic incontinence, palmar and axillary hyperhidrosis, chronic anal fissure.
Intrasphincteric injection of botulinum toxin seems to be a reliable option providing temporary alleviation of sphincter spasm and allowing the fissure to heal.
Injection of botulinum toxin, however, is a simple noninvasive technique that avoids the greater risk of incontinence (which is present in 12 – 35% after undergoing surgery for anal fissure).
Treatment with botulinum toxin produces a quick and more effective alleviation of pain with healing of chronic anal fissures in children also. The treatment is not considered to carry any risks but requires light anaesthesia..
